Case Management Services
Case Management is a service provided to patients, families and guests of St Joseph’s as well as the communities we serve.
The Case Management Department is located in the hospital, but reaches far beyond the hospital walls to meet the medical
and social needs you may have as you experience changes in your health. Case Managers are available to help you identify
and meet those needs. Case Management’s registered nurses and social workers provide services to all in-patient areas
and many out-patient units. These services involve an interdisciplinary team consisting of nurses, physicians, social workers, physical therapist, and many other who will assist you during your hospital stay. As a valued patient, we feel it is essential for
you to participate in your discharge plans. Your Case Manager will work with you and the health care team to assist you in discharge planning whether you are going home or to another level of care. Services involving discharge planning include helping to determine the need for home care services, medical equipment, home health, hospice services, rehabilitation services, community nursing homes, assisted living, psychiatric services, and other outpatient services. We can provide information and literature to community links such as Adult and Aging Services, Children and Family Services, Lifeline, Medicaid, Shelters, Drug and Alcohol Treatment Centers, Counseling Centers, and other community services. Case Managers also work with your insurance company to ensure your benefits are being used wisely in order for you to continue to have benefits when you need them. Patients with Limited English Proficiency, Deaf or Hearing Impaired, Blind or Visually Impaired, Mentally Disabled or Learning Disabled will be offered alternative ways of communicating. The hospital contracts with ATT Language Line, a telephone interpretation service, uses qualified sign language interpreters, amplification devices, telecommunication devices for deaf persons, and other assistive communication devices.
